If you or your family feel your rights as a patient/parent were not upheld, you have the right to voice a grievance and receive a written or verbal response. Whenever possible, ZÖe Pediatrics attempts to resolve patient questions or concerns quickly and informally. If this is not possible, patients/parents are afforded the opportunity to file a formal grievance. Because physicians and staff members are usually the first point of contact for patients, all ZÖe Pediatrics employees have an understanding of the organization’s grievance procedures.

Please fully explain the essential facts of this complaint. What service did you not receive? What was wrong with the service received? What billing issues do you have? Explain who, what, where, when, and how. Your grievance must be in writing, containing the name and address of the person filing it. We will not retaliate against you for filing a complaint.
